How do you get rid of deep wrinkles on the face?

Deep wrinkles are rarely solved by one treatment, because they combine muscle movement, volume loss and weakened collagen. Doctors usually relax overactive muscles with botulinum toxin, support folds with hyaluronic acid filler, and rebuild collagen with devices such as Ulthera, Thermage FLX or Sculptra. Filler effects are immediate, toxin takes 1–2 weeks and collagen improvement builds over 2–3 months. Deep wrinkles soften rather than disappear, and results vary.

Sessions
1–3 depending on the combination
Interval
Toxin every 3–4 months; devices yearly
Downtime

Understanding deep wrinkles

Why some lines become etched

Deep wrinkles are static creases that remain visible even when the face is at rest. They form over years as collagen and elastin decline, fat pads thin or descend, and repeated muscle movement folds the same skin again and again. Sun exposure, smoking and genetics speed this up.

They commonly appear as forehead lines, frown lines between the brows, nasolabial folds and marionette lines. Each area has a different cause, so each needs a slightly different approach.

Signs of deep wrinkles

  • Lines remain visible with a relaxed face
  • Creases do not fade after moisturiser or rest
  • Folds that cast a shadow under overhead light
  • Lines accompanied by sagging or volume loss
  • Previous skincare alone has made little difference
Deep folds are often linked to sagging skin — lifting the tissue can soften a fold more naturally than filling it alone.
Our approach

Treating every layer

We match each wrinkle to its main cause — movement, volume or laxity — and combine treatments accordingly.

01

Xeomin botulinum toxin

Relaxes the muscles that crease the forehead, brows and eyes so etched lines can gradually soften.

02

Hyaluronic acid fillers

Juvéderm, Restylane or Belotero support deep folds immediately. Soft, fine fillers are used for etched lines in thin skin.

03

Sculptra collagen stimulation

Poly-L-lactic acid gradually rebuilds collagen and volume over several sessions — see our Sculptra page.

04

Ulthera & Thermage FLX

Focused ultrasound and radiofrequency lift and tighten lax tissue that deepens folds, with results developing over 2–3 months.

05

Potenza & CO2 fractional

Microneedling RF or fractional laser resurfaces the skin to improve deep, textured creases.

06

Rejuran & skin boosters

Improve skin thickness and elasticity so results from other treatments look smoother and last longer.

What to expect

Softer folds, natural expression

Drag the handle to compare. Deep creases ease in stages as muscles relax, volume is supported and new collagen forms.

Is it right for you?

Good candidates

  • Static lines on the forehead, brows or around the mouth
  • Folds that are accompanied by volume loss or sagging
  • People wanting improvement without surgery
  • Those who can return for a review at 2 weeks, or continue care at home
  • Adults in good general health with realistic expectations
Very deep folds with significant skin excess may be better addressed surgically — we will tell you honestly if that is the case.
Safety first

Possible side effects

Common, temporary effects include swelling, redness, tenderness and bruising at injection or treatment sites. Botulinum toxin can occasionally cause temporary brow or eyelid heaviness or asymmetry. Fillers carry uncommon risks of lumps, infection and, rarely, vascular occlusion, which can affect skin or vision.

To reduce risk our doctors use detailed facial anatomy, cannulas where suitable, slow low-pressure injection and conservative volumes, and keep hyaluronidase on hand to dissolve hyaluronic acid filler if needed.

We do not inject during pregnancy or breastfeeding, over active infection, or for patients with certain neuromuscular or autoimmune conditions.

FAQ

Deep Wrinkles questions, answered

Can deep wrinkles be removed permanently?
No treatment stops ageing, so results are not permanent. Deep wrinkles can be visibly softened, and regular maintenance keeps them looking smoother over time.
Is filler or Botox better for deep wrinkles?
They do different jobs. Botulinum toxin relaxes the muscles that fold the skin, while filler supports lost volume. Lines on the upper face often respond to toxin; folds on the lower face usually need filler or lifting. Many plans use both.
How long is the recovery?
Most patients return to normal activities the same day. Swelling or bruising, if any, usually settles within 2–5 days. Lifting devices typically leave only mild redness.
I'm visiting Seoul for one week. What can I do?
A week is enough for toxin, filler and a lifting device session. We suggest treating early in your stay so we can review you before you fly.
Does treatment hurt?
Numbing cream and fillers containing lidocaine keep injections comfortable. Ulthera can feel warm or briefly sharp, and settings are adjusted to your comfort.
What if I only have early lines?
Shallow lines often need lighter treatment — see our fine line treatment page.
