What is the most effective treatment for smile lines?

Smile lines (nasolabial folds) usually respond to a combination: hyaluronic acid filler to soften the fold, plus support for the mid-cheek, where volume loss often deepens the line. At Nest Clinic we may add Sculptra for gradual collagen or ultrasound lifting such as Ulthera for laxity. Filler effects are visible immediately and typically last 9–18 months; results vary with skin quality, age and lifestyle.

Understanding smile lines

Why the fold deepens

Smile lines run from the sides of the nose to the corners of the mouth. Everyone has them to some degree — they are where the cheek meets the lip. They become deeper with age as the mid-cheek fat pads lose volume and descend, skin collagen thins and ligaments loosen.

Because the cause is often above the line itself, filling only the crease can look heavy. Our doctors assess the cheek, jawline and skin quality together, and also look at related concerns such as marionette lines and loss of facial volume.

Signs your smile lines may benefit from treatment

  • The fold stays visible when your face is relaxed
  • Make-up settles into the line by midday
  • Your cheeks look flatter than they used to
  • The lower face looks tired or heavier in photos
  • Lines appear deeper on one side than the other
Very deep folds often relate to sagging skin; in that case lifting first and filling second usually gives a more natural result.
Our approach

Treating smile lines in layers

Your doctor chooses from these options depending on how deep the fold is and what is driving it.

01

Nasolabial filler

Small amounts of a soft, flexible hyaluronic acid filler such as Juvéderm, Restylane or Belotero placed along the fold to soften it while keeping movement natural.

02

Mid-cheek support

Restoring volume high on the cheek lifts the tissue that falls into the fold — often more effective than filling the line itself. See cheek fillers.

03

Sculptra collagen stimulation

Poly-L-lactic acid builds your own collagen gradually over 2–3 sessions for a subtle, longer-lasting improvement in support.

04

Ultrasound lifting

Ulthera or Ulthera Prime targets the deeper SMAS layer to tighten lax tissue around the lower face, with gradual change over 2–3 months.

05

Radiofrequency tightening

Thermage FLX, XERF or Oligio heat the dermis to firm skin and improve crepiness around the mouth.

06

Skin quality boosters

Rejuran or Skinvive improve hydration and elasticity so the surface line looks softer and fine creases are less visible.

What to expect

A softer, natural fold

Drag the handle to compare. The aim is a gentler transition from cheek to mouth, not an erased or flat smile.

Is it right for you?

Good candidates

  • Moderate smile lines visible at rest
  • Mild mid-cheek volume loss
  • Early lower-face laxity
  • Visitors who want a subtle change with little downtime
  • Patients who prefer a reversible first step
If you have had permanent fillers or implants in the area, please tell us — it changes which options are safe.
Safety first

Possible side effects

Swelling, tenderness, redness and small bruises at injection points are common and usually settle within 3–7 days. Uncommon risks include lumps, asymmetry, infection and delayed inflammatory reactions.

A rare but serious risk of any filler is blockage of a blood vessel. We reduce risk with detailed anatomical knowledge, cannula techniques where appropriate, slow low-pressure injection, and hyaluronidase on hand to dissolve hyaluronic acid filler immediately if needed.

We postpone treatment during pregnancy or breastfeeding, with active skin infection or cold sores near the area, or with certain autoimmune conditions — your doctor will review your history.

FAQ

Smile Lines questions, answered

Will filler make my face look puffy?
Not when the amount and placement are conservative. We often treat the mid-cheek rather than overfilling the fold, which keeps the face natural when you smile.
How long do smile line fillers last?
Hyaluronic acid filler in the smile lines typically lasts 9–18 months, depending on the product, your metabolism and how expressive your face is. Sculptra and lifting results build gradually and usually last longer.
Can I fly home after treatment?
Yes. Most patients can fly the next day. Swelling may be slightly more noticeable on a long flight, so we suggest keeping hydrated and avoiding alcohol for the first 24–48 hours.
Does it hurt?
We apply numbing cream and most fillers contain lidocaine, so most patients feel only mild pressure. Ultrasound lifting can feel more intense along bony areas; we discuss comfort options beforehand.
Filler or lifting — which do I need?
If the fold is mainly from volume loss, filler or Sculptra helps most. If skin is lax and drooping, Ulthera lifting or radiofrequency often comes first. Many patients benefit from both.
Can it be reversed if I don't like it?
Hyaluronic acid filler can be dissolved with hyaluronidase. Sculptra and device treatments cannot be reversed, which is why we start conservatively.
