Can under-eye bags be removed without surgery?

It depends on the cause. Bags from protruding fat pads can only be removed surgically, with lower blepharoplasty. Non-surgical treatment works by camouflage and support: hyaluronic acid filler blends the hollow beneath the bag so the step is less visible, while radiofrequency tightening and Rejuran improve thin, crepey skin. Mild to moderate bags often look noticeably smoother, but results vary and larger bags may need a surgical referral.

Understanding under-eye bags

Where the bulge comes from

Under-eye bags usually form when the fat that cushions the eye pushes forward as the supporting membrane and skin weaken with age, genetics or sun damage. The groove below the bulge — the tear trough — deepens at the same time, so the bag casts a shadow that makes the eye look tired.

Bags are different from puffy eyes, which come and go with fluid, salt, sleep or allergies, and from dark circles caused by pigment or thin skin. Many people have a combination, so diagnosis comes first.

Signs of structural eye bags

  • A bulge that is present all day, not only in the morning
  • A visible groove or shadow below the bulge
  • Bags that look larger when you look upwards
  • Runs in your family or appeared in your twenties or thirties
  • Thin, crepey skin over the lower lid
If the fat bulge is large, filler can make the area look heavier. In that case our doctors will tell you honestly and suggest a surgical opinion instead.
Our approach

Soften, support and smooth

Non-surgical care improves how bags look rather than removing fat. Your doctor combines these options only where they suit your anatomy.

01

Tear trough blending

Small amounts of soft hyaluronic acid filler under the bag smooth the step between lid and cheek. See our under-eye fillers page for details.

02

Cheek support

Where mid-face volume has dropped, carefully placed cheek fillers can lift the lower lid junction and reduce the shadow.

03

Potenza RF microneedling

Radiofrequency microneedling on the lower lid skin firms thin, crepey texture over several sessions.

04

Rejuran healer

Polynucleotide injections improve skin quality and elasticity under the eyes, making the area look less tired.

05

Honest referral

When surgery is the right answer, we explain why and help you find a surgical opinion rather than over-filling.

What to expect

A smoother lid–cheek junction

Drag the handle to compare. Filler blending softens the shadow beneath the bag rather than removing it.

Is it right for you?

Good candidates

  • Mild to moderate under-eye bags with a visible groove
  • Thin or crepey lower-lid skin
  • People who prefer to avoid or delay surgery
  • Visitors who can stay a few days for any swelling to settle
Large fat bulges, significant skin excess or eyes that swell easily are usually better suited to a surgical opinion.
Safety first

Possible side effects

Swelling, tenderness and bruising are common after under-eye filler and usually settle within a week. Uncommon risks include lumps, a bluish tint if filler sits too superficially (the Tyndall effect), prolonged swelling and infection. Rarely, filler can block a blood vessel, which can cause skin damage or vision problems.

We use cannula technique where appropriate, small volumes, soft fillers and a thorough knowledge of anatomy, and keep hyaluronidase on hand to dissolve filler if needed.

We do not inject during pregnancy or breastfeeding, over active infection, or for patients with a history of severe allergy to filler components.

FAQ

Under-eye Bags questions, answered

Will filler make my eye bags disappear?
Filler does not remove the bag; it smooths the hollow beneath it so the bulge is less noticeable. For mild to moderate bags this can look clearly fresher, but large bags usually need surgery.
How long is the downtime?
Most people have mild swelling for 2–3 days and sometimes bruising for up to a week. We suggest scheduling filler early in your stay.
Can I fly soon after under-eye filler?
Usually yes, after 24–48 hours. Swelling can feel slightly more noticeable on a long flight, so keep hydrated and avoid alcohol. We give you a written summary of the product used.
How long do results last?
Under-eye filler often lasts around 9–12 months, sometimes longer, because this area moves little. Skin-quality improvements from Potenza or Rejuran fade gradually and benefit from maintenance.
What if I do not like the result?
Hyaluronic acid filler can be dissolved with hyaluronidase. Our doctors use conservative amounts so adjustments are rarely needed.
Are my bags or my dark circles the problem?
Often both. Pigment-related dark circles need toning or brightening care rather than filler, so we check carefully.
