Do fillers work for marionette lines?

Yes, in most cases. Soft hyaluronic acid filler placed beneath marionette lines softens the groove and supports the mouth corners, so the face looks less tired or sad at rest. Treatment takes about 20 minutes, results are visible immediately and typically last 9–12 months. Because these lines are also caused by mid-face descent and muscle pull, the most natural results often combine filler with cheek support or botulinum toxin.

Understanding marionette lines

Why the lines appear

Marionette lines run from the corners of the mouth towards the chin. They form as the fat pads of the mid-face and jaw lose volume and descend, skin loses elasticity, and the muscle that pulls the mouth corners down becomes relatively stronger.

Treating only the groove can look heavy, so we look at the whole lower face — including nasolabial folds, early jowls and sagging skin — before deciding where filler belongs. You can read more about the condition on our marionette lines page.

What marionette filler can improve

  • Lines running down from the mouth corners
  • A sad or tired look at rest
  • Hollowing beside the chin (pre-jowl area)
  • Downturned mouth corners
  • Shadowing along the lower face
If downturned corners are the main concern, see mouth corner lift fillers, which focus on small, targeted support at the corners themselves.
Our approach

Support from several angles

We use soft, flexible fillers such as Restylane, Belotero or Juvéderm that move naturally with speech and expression.

01

Marionette filler

Soft filler placed beneath the groove to smooth the line and lift the shadow.

02

Pre-jowl support

Small amounts beside the chin smooth the jawline and reduce the look of early jowls.

03

Mid-face support

Restoring cheek volume can reduce the descent that deepens marionette lines from above.

04

Muscle balancing

A small dose of Xeomin relaxes the muscle that pulls the mouth corners down.

05

Lifting devices

When laxity is significant, lifting such as Ulthera or Oligio is combined with a lighter filler dose.

What to expect

A softer, more rested lower face

Drag the handle to compare. The aim is a calmer resting expression rather than a filled-out lower face.

Safety first

Risks & how we reduce them

Common, temporary effects are mild swelling, tenderness and occasional bruising for a few days. Less common risks include lumps that can be felt when smiling, asymmetry, a heavy look if overfilled, infection and rare vascular complications.

We reduce these with soft fillers, cannula use where appropriate, conservative volumes and doctor-only injection. Hyaluronidase is kept on hand so filler can be dissolved if ever needed.

We do not inject during pregnancy or breastfeeding, with active infection near the mouth, or soon after dental treatment.

FAQ

Marionette Fillers questions, answered

How long do marionette fillers last?
Usually 9–12 months. The mouth area moves a lot, so filler is absorbed a little faster here than in the cheeks or chin.
Will filler completely remove the lines?
Filler softens the lines and the shadow, but deep or long-standing lines usually improve rather than disappear. Combining support techniques gives the most natural result.
Does it hurt?
Numbing cream and lidocaine-containing filler keep discomfort mild. Most patients feel pressure and brief stinging.
Can I fly home the next day?
Yes, generally. Avoid strong facial massage, saunas and intense exercise for a couple of days.
Filler or lifting for marionette lines?
Filler restores lost volume; lifting treatments tighten loose skin. Many patients have both, and your doctor will suggest the balance that suits your face.
